SMART Triage System


The SMART Triage Tags are carried in a zippered pouch that attaches to your belt.


Use the RPM or START method of triage. A quick reference guide to that is attached to the inside of the zipper pack. There’s also a quick reference to JUMP START for pediatric triage.


To triage a patient, remove the tag from its plastic pouch, fold it so that the desired triage level and color are on the outside and place it back in the plastic pouch. Use the rubber band to attach the pouch and tag to the patient’s wrist.

There’s space for LOTS of details on the inside of the folded triage card. We will use this space to record procedures and treatments carried out in the treatment sector, if we have a treatment sector running.


The tear-off tabs are used to track where patients were transported to.

Black tags are separate from the folding cards. Attach the black tag directly to the deceased victim.

To warn others of contamination on patients or victims, use the folding cards in the pack. Fold it so that the appropriate contamination category is visible and complete the check-boxes. The folded card slips into a slot on the outside of the triage tag pouch so it’s visible.
This will probably be done at treatment or transport and not in the field.

Small red light sticks are in the pack. At night, active the light stick by “breaking” it and leave one at each red-tagged victim. This makes it easy for those fetching victims to locate the red tag victims first.
